- Church Health Forms

- Each year, you are required to evaluate the health of your church.
- Church Health Form - National 2024 CLICK HERE. - This is your final results of your church health assessment form. Use the compiled Council results to fill out.
- Church Health Form – Council 2024 CLICK HERE – This is a Working Copy of your church health form. Send this to your Church Council. Once they submit them, they will come back to the Lead Pastor to compil. Use them to fill out the National version. We will receive this version at the National Office.
