Mail the following to CRA:

•    T1223 Form
•    T1213 Form
•    Copy of Ordination Certificate
•    Copy of Job Description

Additional Information may be requested by the CRA:

•    Letter of verification for fair market rental value of home
•    Letter of verification of personal housing costs or rental agreement
•    Letter of employment verification from church

The CRA will send an approval letter directly to the applicant and not the church.  A copy of the approval letter is to be provided to the church before payroll can be adjusted for CRD.  If the approval is not received within a timely manner, the church should suspend the calculation of CRD until letter is received.
