Termination of Coverage

Your coverage terminates under this Group if you cease to be eligible due to leave of absence, age limitation or retirement, change of classification, if you terminate your employment or if the Group terminates, etc. Written notification of employee termination is to be provided to the FBP Administrator, at the National Office, prior to the last day of employment.
